LUTS：下尿路症状。包括尿频、尿急、尿失禁等一系列临床表现。常见于老年男性,对患者的生活质量有负面影响。由于下尿路症状常与潜在的前列腺疾病伴发,使得对该病的治疗更加复杂。在由膀胱功能障碍导致的男性下尿路症状中,应特别强调膀胱过度活动症,它以尿急为特征,伴或不伴急迫性尿失禁,常伴有尿频和夜尿。与下尿路症状一样,膀胱过度活动症的发生率随年龄增加而增加。男性膀胱过度活动症常由膀胱功能障碍(如逼尿肌过度活动和逼尿肌收缩力受损)和(或)膀胱出口梗阻引起。逼尿肌过度活动常引起膀胱过度活动症,其尿动力学特征为膀胱充盈期逼尿肌不自主收缩。在评估男性下尿路症状时,症状评分和尿动力学结果应被视为两个相对独立的指标。事实上,膀胱过度活动症(如逼尿肌过度活动)比前列腺异常(如膀胱出口梗阻)能更好地预示膀胱功能异常。因此,在治疗膀胱过度活动症时应考虑可能存在膀胱功能异常。尽管下尿路症状并不确切提示良性前列腺增生或膀胱出口梗阻,但当前治疗下尿路症状(包括膀胱过度活动症)的药物仍然主要针对前列腺。α1受体阻滞剂和5α还原酶抑制剂可以有效治疗有膀胱出口梗阻患者的下尿路症状,但这些药物常不能缓解膀胱过度活动症,对有贮尿期下尿路症状症状的患者可能不是最合适的治疗方法。逼尿肌过度活动和膀胱过度活动症之间的关系明确提示膀胱功能异常在贮尿期下尿路症状发生中的作用。研究提示,抗胆碱药物单独应用或与α1受体阻滞剂联合应用,均可有效改善膀胱过度活动症,不论有无膀胱出口梗阻。从理论上讲,抗胆碱药物对逼尿肌收缩有抑制作用,膀胱过度活动症患者可能存在膀胱出口梗阻,因此抗胆碱治疗可能会加重排尿困难或导致尿潴留。但研究显示,有膀胱出口梗阻和逼尿肌过度活动的患者接受托特罗定治疗12周,最大尿流率无变化,最大尿流率时逼尿肌压力减小,未增加急性尿潴留危险。
